Send the Form to a College Official

Forward the form to an appropriate official at your current or most recent college—this may be:

  • The Dean of Students
  • Judicial Affairs
  • The Registrar
  • Your Academic Advisor
  • Another official with access to your academic and disciplinary records

The college official must:

  1. Complete the second half of the form confirming your academic and social standing.
  2. Submit the completed form directly to Florida Southern.
