Immunizations
Before you move into your residence hall and begin classes, all students, regardless of age, must submit immunization records for review. Here's what you need and how to submit it.
Required Immunizations
All students must have the following immunizations:
- Td (Tetanus/Diphtheria) and/or Tdap (Tetanus/Diphtheria/Pertussis)
- Varicella (Chickenpox)
- Hepatitis A
- HPV
- Polio
- Meningitis B
How to Submit Your Records
Florida Southern partners with Med+Proctor to verify and manage student immunization compliance. All students, regardless of age, must submit records through Med+Proctor.
Submit early. Late, incomplete, or inaccurate records will delay your move-in to the residence halls and may prevent you from attending classes.
Steps:
- Create an account at medproctor.com using your personal email as your ID.
- Complete the required steps, including confirming your email address.
- Have your healthcare provider complete your immunization certificate or form.
- Upload your proof of immunization to your account.
Florida Southern accepts any of the following as proof:
- The Med+Proctor Immunization Certificate
- The FSC Immunization Form (PDF)
- The official State of Florida Immunization Form
- Local health department forms
- Stamped high school transcripts
- Military immunization forms
- Physician's office records
A note on fees: Med+Proctor offers an optional service to access state immunization forms directly and enroll in lifetime record access for a one-time $10 fee. This is optional and not required to meet FSC's requirement.
Waiving a Vaccine
To request an exemption, submit a completed Religious or Medical Exemption Release Form (PDF) to Med+Proctor. Students under 18 must have the form signed by a parent or guardian.
